16 years old or older.
15 years old or younger.

Do any of the following apply to the student?

Has a current IEP or 504 plan in place.
Received special education services in the past.
Suspect the student has a physical or emotional condition that could be a barrier to employment but does not qualify for, or has refused, special education services.

Do you anticipate the student would benefit from assistance with any of the following?

Getting and keeping a job.
Pursuing postsecondary education/vocational training.
Services to evaluate and address barriers to employment (e.g., accommodations, assistive technology, etc.).

Is the student interested in pursuing DVR service?

Student has indicated a willingness.
Student needs more information before committing.
